The problem
The form asks for the date, and the card is in a drawer somewhere.
School wants a date. Camp wants a date. The new pediatrician wants all of them. The yellow card is in a drawer, or a folder, or the last house, and the clinic that gave the second dose closed.
So you guess, or you spend an afternoon on the phone, once a year, forever.
What's inside
One row for every dose anybody has had.
Type each dose once: who it was for, which vaccine, the date, which dose in the series, where it was given, and when the next one is due. The file counts down to the next dose for each person and marks anything already overdue.
It is a copy for your own use. It is not a legal record, and it does not replace the card or the clinic’s file.
Vaccinations. One row for each dose, with the person, the vaccine, the date given, the dose number, where it was given and the next one due.
The countdown. Days to the next dose, worked out for every row, so a date that has passed cannot hide.
The Dashboard. The next dose due and whose it is, how many people the file covers, how many are up to date and how many doses are overdue.
Red and gold. A dose past its date turns red. One coming up turns gold.
Set up. Your own people, vaccines and places, behind the dropdowns on Vaccinations.
Sample records. Twelve made-up doses across four people, so the dates make sense before you type your own.
What it looks like

Questions
Before you buy.
Will a school accept this?
Treat it as your own copy, not as proof. Schools and clinics want the official record. Keep the card. This is how you find the date without it.
Does it tell me which vaccines we need?
No. It holds the dates you put in it and counts down to the next one you entered. What is due, and when, is between you and your clinician.
Can I keep the whole family in one file?
Yes. Every row says who it is for, and the Dashboard counts the people, the ones up to date and anything overdue.
S
The date is in the file, not in a drawer.
No app to learn. No new account. No monthly anything.
